SAR DFP Conflict of Interest Disclosure

SAR DFP Members

All DFP chairs and panelists (including consultants and non-members) must complete the SAR DFP Conflict of Interest disclosure form (COI) annually.  After the deadline, the forms will be archived until they are needed for a collaboration or project representing the SAR, at which time we will cross-check and verify that all have been submitted.

Any questions may be directed through panel Chair(s).

Deadline to complete disclosure form:

June 30, 2017

The Society of Abdominal Radiology (SAR) requires that prior to participating in any Diseased-Focused Panel (DFP) publications, all individuals make full disclosure of relationships, business transactions, and presentation or publications related to healthcare or SAR activities. All relevant items for the last 12 months should be disclosed.

Financial relationships are those relationships in which individuals benefit by receiving a salary, royalty, intellectual property rights, consulting fee, honoraria, ownership interest (e.g. stocks, stock options, or ownership interest, excluding diversified mutual funds), or other financial benefit. Financial relationships consist of employment, management position, independent contractor (including contracted research), consulting, speaking and teaching, membership on advisory committees or review panels, board membership, and other activities from which renumeration is received or expected.

Relationships should be reported regardless of whether or not compensation was received.

All relationships in commercial interests should be reported. A commercial interest is any entity producing, marketing, re-selling, or distributing healthcare goods or services consumed by, or used on, patients.
