SAR DFP Conflict of Interest Disclosure

SAR DFP Members

All DFP chairs and panelists (including consultants and non-members) must complete the SAR DFP Conflict of Interest disclosure form (COI) annually.  Non-panelist collaborators participating in DFP projects are also required to complete the form during the year(s) of active collaboration.

Each year at the time of annual panel reports, submitted forms will be archived until they are needed for a collaboration or project representing the SAR, at which time we will cross-check and verify that all have been submitted.

You may contact the SAR office for a list of submissions at any time. Additional questions may be directed through panel Chair(s).
