DFP Survey Request Form

All requests must be submitted electronically via the form below.
Requests may come from any DFP member. Be sure to review the form and have all required information ready.  Forms cannot be edited once submitted, but a follow-up email is acceptable if you have updates. 


SAR policy allows for the DFP Oversight Committee to approve up to two (2) surveys per year. Each survey will be sent via email through the SAR office to all current SAR members,

and will include a dedicated, personalized email blast and up to two (2) reminders.  The survey must be built and managed by the Panel, with all responses going directly to the panel, not through the SAR office. 


If a member survey is integral to the goals and progress of your Panel, please submit your

request online as soon as possible.  Based on the requests received, the DFP OC will select the surveys and coordinate the timing of each.



Survey requests may be submitted at any time, and are reviewed and considered on an ongoing basis, as they are received. The final date to submit a request for a survey to be sent prior to the next upcoming annual meeting is November 1st
