New Panel Applications:

Panels should contact the DFP/ETC Oversight Committee and begin working with an OC member prior to the application deadline to ensure a successful application.  


DFP/ETC Room Request Form (during RSNA):

The SAR will rent a meeting room during RSNA, specifically for Disease-Focused Panels. The room will be available to all Panels for a set period of time, on a first-request basis, on a single day (Tuesday). If you would like to take advantage of this offer and meet in the SAR-DFP/ETC room, please submit your TOP 3 preferred times. The SAR will cover the room rental fee only; A/V, Food & Beverage or other arrangements will be at the panel's expense.


Survey Request:

Survey requests may be submitted at any time, and are reviewed and considered on an ongoing basis, as they are received. The final date to submit a request for a survey to be sent prior to the next upcoming annual meeting is noted in the timeline above. SAR policy allows for the DFP/ETC Oversight Committee to approve up to two (2) DFP/ETC-related surveys per year.  Follow the form link for additional information about DFP/ETC surveys to the SAR membership.


New Panel Application Re-submissions:

For potential new panels working toward acceptance, revised applications should be submitted through the DFP/ETC OC liaison working with you.  There is no special form.  


DFP/ETC Meeting Registration (during SAR Annual Meeting):

Active panels are required to meet together during the SAR annual meeting.  Panel Chairs are also required to attend the Panel Chair Town Hall meeting.  New Panels are required to meet with the DFP/ETC OC for an individual appointment,and active panels are welcome to schedule appointments as needed.   Please visit the DFP/ETC Meeting Schedule page for additional information and registration forms. 


Consultant Reimbursement Form:

Consultant reimbursement forms and receipts can be completed and returned online or using the downloadable form.  With any questions, please contact the DFP/ETC administrator in the SAR office.  


Annual Panel Report Form:

Following the annual meeting, all active and approved panels must submit an annual report using the online form. Carefully review the form for new items and requests, including: inter-society and international relationships; collaborations; updated panel rosters; website edit requests; member survey requests.

All rosters must be submitted using the standard roster template.


DFP/ETC COI (conflict of interest) Form:

All DFP/ETC chairs and panelists must complete the SAR DFP/ETC conflict of interest form (COI) annually. Panel chairs should distribute this link, request participation of all members of the panel and follow up accordingly. After the deadline, the SAR will archive the COIs until they are needed for a collaboration or project representing the SAR, at which time we will cross-check and verify that all have been submitted.


MOU (memorandum of understanding) Template: please contact the DFP/ETC Oversight Committee through the SAR office for the latest MOU template and collaboration guidance information.  An MOU, and in some cases, an SOW (statement of work), will be required for all SAR collaborations with other societies, organizations, companies, etc.  Each MOU should be developed with the collaborating organization using the SAR template and submitted to the DFP/ETC OC for review and for facilitation of board approval.
